About Shared Lives Stoke
Shared Lives Stoke is a unique person-centred service that enables adults with a learning disability to live within our communities, sharing their live with an ordinary family. An approved Shared Lives Carers will welcome an adult with learning disabilities into their home, this could be for a few nights or something more long-term they will support the person to be independent and achieve their potential
We will match the person wishing to access the service and the Shared Lives carer to make sure they have common interests and lifestyles. This helps to ensure the arrangements works for everyone, if people aren't happy with the match we will look for another match. If we can, we like to offer a choice.

- would be over 18
- has care and support needs as assessed by their social worker
Shared Lives Carer can support with:
- personal care
- medication
- medical appointments
- daily living skill
- providing transport to leisure activities, work placements and accessing the community
- holidays and outings
- family visits and contact
- meal preparations
