You can use our online care and support self-assessment tool if you would like to make a self-assessment of your own care and support needs.

Who is the self assessment tool for?

The tool is for those aged 18 years or over who are seeking information and advice about care and support needs.

If you are a friend, relative or support organisation that is supporting a person to get the help or information they need, you can complete the assessment with that person. It is important that you have their consent and ensure they are fully involved in completing the assessment.

What does the self assessment tool do?


  • The tool asks you to provide details about the care and support needs and provides you with a wealth of helpful information, advice and guidance that can be used.
  • The tool also provides a recommendation as to whether a request for an actual care and support needs assessment might be helpful. Details of what this entails and how to prepare yourself can be found here.
  • If a care and support needs assessment is recommended, the tool can be used to easily send your information on to us at the local authority.  An advisor will  then be in touch shortly to arrange this.

  • If after using the tool you don't think a care and support needs assessment is needed, you can use the information, advice and guidance to support yourself or the person you care for further.

I have requested a needs assessment, what happens next?

Initially, when we speak to you we can offer you some information, advice and guidance that you can use to determine if you do actually need a care and support assessment. There are many ways we may be able to help you or point you in the right direction.

If you still want a care and support needs assessment, then we will take some details and can put a request straight through for you.

After a request has gone through, you will be contacted by the social care team and you will have a conversation to build an understanding of your situation and your care and support needs. Details of what this entails and how to prepare yourself can be found here.

Following this process, a decision will then be made about your eligibility for local authority funded care and support.
