Process to becoming a Small Support Provider...

There are 3 stages in the Process

STAGE 1: FACT FINDING AND ENQUIRIES…

First, we need to find out if Small Supports is really for you.

If you feel that you meet the criteria above, please contact us on the details at the bottom of this page. We will get in touch with you and explain the requirements in further detail.

If you decide to move forward, we will ask you to express interest in writing, explaining why Stoke-on-Trent should invest in you and how you will contribute towards transforming social care.  

Following that, if you share our vision and values, we will arrange an interview with a number of Health and Social Care representatives to find out more about your experience and motivation behind wanting to become Small Supports Provider.

Finally, if you're successful, we will get in touch with you and you will be invited to meet Experts by Experience, who will build a connection with new Providers becoming their role models.

The Small Supports care provider will be paid by the individual that they will be supporting via Direct Payments or Independent Service Funds. 
STAGE 2: APPROVAL PROCESS…

Once approved, we will support you to register with the Care Quality Commission (CQC) to deliver personal care services in the local area.

Our project team will be on hand to guide you through the process – starting from registering your organisation with Companies House.

STAGE 3: ONGOING SUPPORT…

Throughout the process, we will work with yourself and our Adult Social Care teams to match your newly registered Small Supports Organisation with an individual who needs support to achieve their outcomes.

We will ensure that appropriate transition arrangements are in place (Stoke-on-Trent City Council to new Small Support Provider) to make the process as effortless for the individual as possible.

We will help you succeed by:
  • Securing Stoke-on-Trent Integrated Commissioning Board support for the project
  • Committing to collaborating with experts across Integrated Commissioning System to develop and implement efficient system improving our community
  • Sharing experience and lessons learned from providers from other parts of the country
  • Providing support of our project team throughout the process
  • Implementing systems and changing culture within Adult Social Care teams, to ensure process is simple, free of unnecessarily bureaucracy, inclusive and supportive.
  • Connecting you with other organisations, which will help you and your organisation flourish
  • Providing a guarantee of matching you and your skills with a person requiring support to achieve their outcomes
