Stoke-on-Trent Adult Social Care views all of our residents as unique individuals who have strengths, assets, gifts and talents. As a result, we have developed a Community Led Support approach to support people to live more independent and fulfilled lives within their local communities.

You can access our Community Directory using the Community Led Support button below. This directory can be accessed at any time and aims to help you to identify the support you may need and link you into possible resources in your local area.
Alternatively you can attend one of our Community Lounges to discuss your situation with a member of the team directly. They will be able to offer you advice and support. These lounges are held at regular intervals in your community. To find out where your local lounge is located and on what days it runs use the Community Led Support button below and click on the Community Lounge Schedule tab.
If you feel that you may require some equipment within your home you can access an online support tool by clicking on the Technology to help you button below which will, based upon your answers recommend possible equipment options and where these can be purchased.
If you are unsure what support you may need, having looked at our Community Directory or you feel that you require support accessing the Community Directory you can ring our Contact Centre on 0800 5610015.

- Have conversations and develop relationships with residents to better understand what you can do, rather than what you can't do.
- Develop help in the community by working together with groups and organisations, supporting them to support you.
- Develop and embrace new ways of working by allowing and supporting our workforce to be creative in providing support.
